Babies and Toddlers on the Couch? Early Intervention with Psychosomatic Illness

 

A case of a sleepless child in post 9/11 New York City

Traumatized mother, sleepless baby: Mother–Infant Psychotherapy with a sleep-impaired 18-month-old boy, whose experience of impaired maternal functioning impacted his early psychic life. The child presented in an agitated state while his mother resembled the proverbial "dead mother," unable to feel any positive emotions. The report of play sessions will show the technique of intervention with the depressed dyad. 

A case of a wordless toddler with severe eczema

Suppression of affects and psychosomatic balance in a dyad: Mother-Child therapeutic intervention with a 2-year-old boy presenting with head-banging and eczema.
Difficulty establishing object constancy and stable mental representations in a young child lead to bodily symptoms and non-pleasurable self-soothing activities. The dyad demonstrated pure arousal without any containing function or clear emotional state.  The mother’s inability to give meaning prevented the child from linking emotion with symbolic activity. But regaining contact with his maternal object made him recover.

 

Learning Objectives

As a result of attending this session, participants should be able to:

  • List multi-layered dynamics present in infant-parent interactions
  • Summarize techniques of intervention when trauma has impaired a mother’s ability to care for her baby
  • Describe approaches to developing the working alliance with parents in order to facilitate in-depth work with young children

Christine Anzieu-Premmereur, MD, PhD, is a child and adult psychiatrist and psychoanalyst in New York City. She is on the faculty at the Columbia Psychoanalytic Center and Director of the Parent-Infant Psychotherapy Program. A member at the New York Psychoanalytic Institute, she chairs the discussion group on Parent-Infant Programs at the American Psychoanalytic Association, and she is the chair of the IPA Committee for Child and Adolescent Psychoanalysis. 

 

 

Photo of Bart BlinderBart Blinder, MD, PhD, is an adult and child psychiatrist and psychoanalyst in Newport Beach, CA. He is a Clinical Professor at UC Irvine, where he was past Director of Eating Disorder Treatment and Research, and the University of Washington. He is a Senior Faculty Member in the Adult and Child & Adolescent Training Programs and the Child & Adolescent Psychoanalytic Psychotherapy Program at the New Center for Psychoanalysis, where he actively participates in the Child Study Group and Chairs the Research Committee. At NCP, Bart has taught sections on early infant and child development, infancy and childhood antecedents of eating disorders, and neurobiologic and psychodevelopment roots of autistic disorder. He has edited major texts in child psychiatry, eating disorders treatment and research, and integrating psychotherapy and pharmacotherapy.
