“Making a Person”: Psychoanalytic Work with Autistic and Psychotic Anxieties in Childhood

Joshua Durban, Training and Supervising Psychoanalyst of The Israeli Psychoanalytic Institute, is an internationally known child and adult psychoanalyst specializing in therapeutic work with autistic and psychotic children, adolescents, and adults. The paper he will present, "'Making a Person': Clinical Considerations regarding the interpretation of anxieties in the analyses of children on the autisto-psychotic spectrum," is an erudite and emotionally moving work about his psychoanalytic treatment over several years of Rafael, a boy with a mixed clinical picture of autism and psychosis.

Historically, child and adolescent patients manifesting autistic and psychotic pathology have been very challenging to treat, and psychoanalytic approaches have been regularly criticized. In this presentation, Joshua Durban reviews the development of certain psychoanalytic trends that have emerged in recent decades that many clinicians find helpful in reaching these patients. After summarizing some of these approaches, Durban presents in depth his analytic treatment of a young adolescent patient with a mixed clinical picture of autism and psychosis. Durban proposes an "autisto-psychotic" level of primitive anxieties and demonstrates the effectiveness of this perspective in the rich clinical material presented.

As a result of attending this session, participants should be able to:

Explain early infantile autistic states and their development throughout adolescence and young adulthood
Discuss the various levels of anxieties, defenses, and unconscious phantasies involved in autistic states of mind
Describe the different technical requirements and interpretive levels involved when working with autistic states of mind
Assess how the total transference-countertransference situation may be utilized to establish contact with the withdrawn patient in order to promote change

Presenter:

Joshua Durban is a Training and Supervising Child and Adult Psychoanalyst at The Israeli Psychoanalytic Society and Institute, where he also teaches. He works in private practice in Tel Aviv with children, adolescents, and adults and specializes in the analysis of ASD and psychotic children and adults. Mr. Durban was one of the first clinicians to psychoanalyze ASD children in Israel. He is Founder and Co-Chair of The Israeli Psychoanalytic Inter-Disciplinary Forum for the Study of ASD, a joint venture of The Israeli Psychoanalytic Society and The Israeli National Health Service. He is also on the faculty of The Sackler School of Medicine, Tel Aviv University, the Psychotherapy Program, the Early Mental States Unit, and co-founded the Post-Graduate Kleinian Studies Program there. Joshua Durban is the Scientific Editor of The Collected Works of Melanie Klein and has articles appearing in such publications as the International Journal of Psychoanalysis, the Journal of Child Psychotherapy, the American Journal of Psychotherapy, and Psyche. He currently serves as Co-Chair of the IPA International Inter-Committee for the Prevention of Child Abuse.
